The ACT Program is open Monday through Friday from 8 a.m. to 4:30 p.m. Its office is in the Butler Rehabilitation Center on the main campus of the Buffalo Psychiatric Center, 400 Forest Ave. The office number is 816-2935. |

Who is eligible:
This program is open to individuals between 18 and 64 who have a primary mental health diagnosis and have had difficulty using services provided by traditional treatment providers (clinics, day treatment programs) in the past. |

What do we offer:
The ACT team provides all aspects of mental health and substance abuse treatment, including psychiatry services. Individuals connected to ACT will not also be served at an outpatient clinic. -Clinical treatment of Mental Illness and Substance Abuse -Assistance with Family Life and Social Relationships -Service Planning and Coordination -Problem Solving Skills Teaching -Daily Activities Skills Teaching -Relapse Prevention -Medication Management -Vocational and Educational Assistance -Healthcare Education and Linkage -Money Management and Entitlement Assistance -Housing Assistance -Peer Support and Assistance. |
